The goal of care management is to achieve an optimal level of wellness and improve coordination of care while providing cost effective, non-duplicative services.
Care management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s health and human service needs. This includes, advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and viable resources to promote quality, cost-effective outcomes.
Our Care Managers assist families with local resources such as home safety, nutrition, Mobile Physicians, Durable Medical Equipment, and Home Modification.
The Care Managers meet with each family to develop a Plan of Care that is tailored for each family respecting the role of the consumer to be a decision maker in the care planning process. Care Plans should be designed to best serve the client and family, be multi-faceted, improve quality and cost effectiveness, and ensure coordination of care. We use evidence-based practices on our interactive care plans that are developed based on consumer-set priorities.
The Care Manager evaluates that the care needs of the client are at the appropriate level of care for our staff. The Care Manager also oversees each care to ensure the highest satisfaction with services and supervision of caregivers.
