Care Choice Home Care team is privately owned and operated by Tara Pardo, Jeani Groesbeck, and Jon Izzo. We have an extensive network of caregivers across Southern California. As owners, we share a combined 64 years of experience in the home care and senior industries.
Tara Pardo, Co-Owner
Tara has lived in North San Diego County for 30 years. She raised her three children in North County and has a great love for and commitment to this community.
Tara has been working in the Home Care industry for over 20 years. She says health care is in her genes, stemming from the long line of physicians and nurses in her family. Her step father practiced Pulmonology in San Diego for 40 years as a partner for Kaiser. He continued to visit his patients in their homes, when needed, until the day he retired. His care and compassion inspired Tara to provide great care for people as well.
She began her career in the early 90’s, working for a local hospital district in their home care department as a Care Coordinator for their skilled Private Duty division. When she took over leadership of the department, she saw the need for a higher quality caregiver in senior’s home, she implemented a non-medical caregiving program.
Tara has been a strong advocate for the home care community. She is a member and serves in leadership positions for the California Association for Health Services at Home (CAHSAH). She participates in lobbying activities that advocate for seniors in Sacramento.
Tara understands the desire for seniors to stay in their homes as long as possible. She believes that every customer should receive individualized treatment with the highest quality customer service, caregivers and care coordination.

Jeani Groesbeck, Co-Owner
Jeani’s involvement in the senior care industry has been a lifelong journey. During her childhood, her father owned a chain of skilled nursing facilities in Michigan prior to making San Diego the family’s permanent home in 1969. As an adult, Jeani and her father founded and operated Southwest Healthcare and Retirement Properties, Inc., specializing in brokering and managing long term care facilities throughout California and the midwest. The partnership continued for over 12 years at which time she stepped back to raise her young family.
In 2007 Jeani was drawn to the home care industry following her mother’s fierce battle against cancer. Though that battle was ultimately lost, her mom’s illness and subsequent passing opened a door that would prove to be Jeani’s future. For the next 7 years home care business development and marketing became her career as well as her passion.
Now, as Co-owner of Care Choice Health Systems, Inc., Jeani is responsible for Business Development, building and nurturing relationships with Clients, their families and community resources. Drawing on decades of experience in the senior care industry, she is committed to identifying gaps in services for older adults and implementing specific programs to meet their needs.

Jon Izzo, Physical Therapist
Following the completion of my graduate education program at the University of the Pacific, Stockton, CA in 1989, I moved to San Diego where I’ve been in PT practice since. My 25 years of experience has placed me in a variety of clinical PT settings. This includes acute inpatient, acute rehabilitation, skilled nursing and home health services. Through the years, I have also had extensive experience in outpatient and private practice settings.
Although I have worked in administrative and supervisory capacities my skills and passion still lie in clinical, hands-on client care.
I have an extensive, well rounded neurologic and orthopedic clinical background with the bulk of my experience in Stroke Rehabilitation, TBI (Traumatic Brain Injury), SCI (Spinal Cord Injury) and diseases of aging.

I have extensive experience in treating neurologic, orthopedic, movement and balance disorders. Kate Stone, R.T., M.A. Administrative Assistant
My career desire is being a DAYMAKER. Essentially, that is someone who makes a difference in her local community. I like being in a position to bring comfort to people, be it easing someone’s everyday stress or helping a person with a life-altering situation.
I grew up surrounded by family members in all areas of the medical field. My education includes graduating top of my class with degrees (and knowledge) in both medical assisting and respiratory therapy. I remain a licensed and registered respiratory therapist with the State of California.
I feel fortunate that my chosen career is also a passion of mine!
